Checks/Money Orders/Bank Notes should be made out to LSU-Eunice and have your reference number listed in the memo line.

Please include a note if you are requesting to be registered in the 12-month payment agreement. This gives you twelve (12) months from the date on your letter before the balance on your account is submitted for collections with the state agency.
